General Mills - Broke 2 teeth as there was a small pebble in the bar.

On March 7, on the first bite of the Bar I broke an upper and lower tooth. This occurred at my home at 82 Old Yankee Rd, Haverhill, Ma.
I made the first appointment I could and saw the dentist on Mar 10. I have the dentist statement, x-rays, a letter written by myself, a photo of the pebble, and the adjusted bill from MetLife. I am requesting the reimbursement of my actual liability. Do you require any additional information before I mail this? Also, what is the most accurate address to send this information?
