Mattress Firm - Purchased a beautyrest pressuresmart queen mattress about

The mattress has started to permanently "sag" and nor return to normal after days of non-use. How do I file a complaint on-line? Please either give an exact link with precise address points for starting. Or advise whether MF can just mail to me or email to me the exact form(s) to complete with all the requested photos.
